The practice is registered with and complies with, the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.

You can now request access to your coded medical record on-line. Even if you already have on-line access, you will specifically have to request access to this service to ensure compliance with on-line security policy.

You will need to complete this form and bring in identification with photo and current address, such as a driving licence or passport and a current bill or bank statement. For further information, please contact the surgery.

Clinical Practice Research Datalink

Information in patient records is important for medical research to develop new treatments and test the safety of medicines. This practice supports medical research by sending some of the information from patient records to the Clinical Practice Research Datalink (CPRD).
