NHS App
The NHS App is a new way for patients to connect to the surgery and other NHS services. For example, you can:
- A – Z symptom checker
- Find out what to do when you need help urgently
- Book and manage appointments at your GP surgery
- Order repeat prescriptions
- Manage your pharmacy nomination
- Securely view your GP medical record
- Review your organ donor decision
- Choose how the NHS uses your data
Download it FREE from the Apple Store (Apple device) or the Google Play Store (Android device)
When you register in the app, there will be a series of checks to confirm your identity. You WILL NOT have to go into your surgery to register if you have a passport or photo card driving licence.
You can choose to log in using fingerprint recognition, facial recognition or username and password of your choice.
You can now also access NHS App services from the browser on your desktop or laptop computer.
Engage Consult
Online consultation using the Engage Consult service.
You can now contact the surgery online to tell us about your non emergency medical problem or ask an admin question. You can ask for help anytime, anywhere within the UK, and with any device. Using Online Consultations allows us to quickly and safely decide on the best way to help you, your child, or an adult in your care.
You must create an account to be able to use this service.

Patient Access
GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:
- Book and Cancel appointments
- Order repeat prescriptions
- Manage your pharmacy nomination
- Get health advice leaflets and videos
- View your GP medical record
You will need a unique ID and password to create an account. Please speak to a member of our reception team next time you call or visit the surgery and they can provide you with this. You will be asked for proof of identify, i.e. a passport, driving licence or proof of address.
Patient access is also available to use as an app. Click on the links below to download to your device.
You can still contact the Practice by phone, this is just another option, which other patients have found is more convenient and saves them time. Proxy Access
You can now request to act as a proxy for children, relatives and dependants that you care for and are patients at Creech Medical Centre using Patient Access or the NHS App.
This enables a parent, family member or carer to act on behalf of the patient with their access tailored accordingly. The proxy feature includes appointment booking, repeat medication requests, text messaging and, where applicable, access to medical records, and can easily be accessed by the proxy once the patient has been linked.
For children under 11 parents or authorised carers should complete a proxy access form. This access becomes restricted to appointment booking and repeat medication requests for minors aged 11-16 and then the rights are automatically switched off when the child reaches 16.
